Appointments

Hours/Referral
Our office is open Monday through Thursday from 8:00 A.M. to 5:00 P.M, Friday 8:00 A.M. to 4:00 P.M. A referral by your Pediatrician is necessary. An appointment for a routine evaluation can usually be obtained within 2-4 weeks; urgent appointments may be obtained within a day (your Pediatrician will need to call our office directly to coordinate this).

What To Bring

1. New patients should arrive at the office at least 15 minutes prior to their appointment in order to complete the New Patient History and Patient Registration forms. To minimize delays, these forms can be downloaded directly from our website and completed prior to your child's appointment.
   
   

2. Please bring your current insurance card and photo ID.
   
   

3. One parent or legal guardian must be present upon first visit. Adopted and/or legal guardian must have legal documentation upon first visit.
   

Hearing Problems
If you think your child has a hearing-related problem or may need a hearing test, please notify the receptionist at the time of your call. A separate appointment with the audiologist can be arranged.

Cancellations
Please call us at least 24 hours in advance if you need to reschedule or cancel your appointment; this will allow another child to see us in a timely fashion.

If our office is not notified, you will be charged $30.00 per missed appointment (this will not be covered by your insurance).

We will be able to bill secondary insurance carriers for surgery ONLY.
